Permission as given on above website: "Should you wish to administer the MFQ to your clients or for your research study, you may download a PDF of any version of the MFQ free of charge. We only ask that you cite the authors in any published work." [Last accessed 31.08.18]
In addition Brian Small granted permission for NHS Scotland to use any version of the MFQ in our EPR systems as long as no fee is charged. Formatting differences in the electronic versions is allowed.
Scoring advice from Brian Small as follows:
"Please remember that there are no prescribed cut points to any version of the MFQ. You are allowed to set your own cut points to fit your situation.
These are not “absolute” cut points for the authors were very deliberate in not prescribing cut points.
What are the implications of a high score? It simply means that the respondent should be further evaluated. Remember, this is only a screen and does not make a diagnosis of depression."
